Here you will find all of the documents needed to successfully enroll your child into Kandy Kane Learning Center. If possible, please take time to go ahead and fill out the paperwork. Please be sure that EACH line is filled. If the line does not apply please put N/A. We look forward to having your child here with us!
We have two options for filling out the documents –
1) You can print the pages off and fill them out. We do ask, that you turn in the forms a few days before your child’s first day.
2) You can also download the forms, fill them out online and they will automatically be emailed to us.
